Centennial School District - Kindergarten Registration

Kindergarten students must be 5 years old on or before August 31, 2011

 To register, please bring the following information to your neighborhood elementary school:

• Birth certificate, baptismal certificate or passport (please bring original and provide a copy for the school to keep)
• Immunization records (please provide a copy for the school to keep)
• Four proofs of residency (include mortgage statement or lease, two utility bills and photo identification)

For children residing in the Leary zone, parents are requested to attend Willow Dale’s registration meeting.

If you are unable to attend the meeting on March 2, 2011, please contact the registration office after March 2, 2011 at 215-441-6000 x 3038 to arrange an appointment for registration.
