Kindergarten Registration Wednesday for Davis Elementary

Registration for Kindergarten students entering Centennial School District will be held Wednesday evening at each of the district's elementary schools.

Parents are reminded that the student MUST be five years of age on or before August 31, 2013. 

Please bring the following required documentation to register your child:

1. Child’s birth certificate: (provide the original and a copy.)
2. Four Proofs of Residency are required. 

• The first is a mandatory proof: a lease, agreement of sale or mortgage statement. 
• Three additional proofs are to include either an electric bill, a telephone bill, a tax bill, a cable bill or a pay stub from an employer.

   3. Child’s record of immunization from the child’s doctor. 

• For attendance in Kindergarten in 2013/2014, children need the following vaccinations:

4 doses of tetanus* (1 dose on or after the 4th birthday)
4 doses of diphtheria* (1 dose on or after the 4th birthday)
3 doses of polio
2 doses of measles**
2 doses of mumps**
1 dose of rubella (German measles)**
3 doses of hepatitis B
2 doses of varicella (chickenpox) vaccine or history of the disease
*Usually given as DTP or DTaP or DT or Td
**Usually given as MMR 

Parents are asked to download the student registration form and the health history form, complete each and bring it to registration.

If you are unable to attend the meeting on March 6, contact the registration office after March 6 at 215-441-6000 x 11046 to arrange an appointment for registration.
